Prevention

Homeless Prevention Program

Prevents homelessness for young adults and young parents (ages 18-24) through life coaching, short-term financial assistance, connection to community resources, and a support network.

Minor Connect

Life coaching for unaccompanied youth experiencing homelessness or at risk of homelessness (ages 15-17).

Homework Starts With Home, Northwest

This program prevents homelessness for families who are experiencing housing instability whose children attend the Brooklyn Center Community Schools and/or Osseo Area Public Schools.

Programming promotes long-term housing stability through goal-oriented case management, supportive services, and financial assistance. 

Intervention

School and Street Outreach

YMCA Outreach Workers work with youth who are homeless or at risk of homelessness at schools and the community to meet immediate needs and connect them to community resources.

This program conducts various housing assessments – Coordinated Entry housing assessment, Coordinated Entry System Assessment authorized in 3 CoCs, trauma screening, and Positive Youth Development assessment – to guide service connection and navigation.

YMCA Youth Resource Line (YRL)

This is a YMCA-managed voice messaging line available 365 days/year from 8 am-8 pm that averages 115 calls from youth in need each month. The YRL is the first step in identifying and assisting youth experiencing housing crises or homelessness. Youth are connected to live support staff for problem-solving and to connect to resources that meet their needs.

Accelerator - Diversion, Rapid Resolution, CES

This program diverts youth from the homeless response system through a blend of problem-solving, housing navigation, Coordinated Entry assessment, and shelter/homeless diversion services. We target youth who are the most disconnected – who are unsheltered or unsupported in the adult shelter system and connect them to youth-specific services and support to find permanent housing solutions.

Safe StaY Emergency Sheltering (Hotel Project)

This program immediately moves youth off the streets into a safe space through our hotel sheltering project with supportive services. This space allows youth time to problem-solve and develop plans for long-term permanent housing solutions with the support of YMCA staff and financial assistance.
